We accept pathology request forms from all healthcare providers.
Please bring the following items to your pathology appointment:
- Your completed pathology request form: this form is required for all tests.
- Your Medicare card (if you have one): if you have a Veterans Affairs Card, please bring that too.
- A list of your current medications and supplements: this helps our collectors and may be important for specific tests.
- Any relevant previous test results (if your doctor has asked you to bring them)
- Photo identification: this is required for certain types of testing, including:
- immigration medicals
- occupational and workplace testing
- parentage/DNA testing
- insurance or legal testing
- drug and alcohol screening
- any court-ordered testing
If your test has special instructions (e.g., fasting, timed collections, stool or urine samples), please follow the directions provided by your doctor or on the request form. You may also bring any home-collected specimens if instructed to do so.
